ORDER FOR EXTRADITION ON WAIVER

            THIS MATTER coming before the Court, on the __________ day of __________________, ________, pursuant to § 31-4-22, NMSA 1978, the defendant, __________________, being present in Court with counsel, __________________; the Court having advised the defendant that he is charged with __________________ in the County of __________________, State of __________________; the Court having advised the defendant of his rights pursuant to § 31-4-10 NMSA 1978, and the defendant, with the advice of counsel, having waived each of the rights therein granted; and the Court being fully satisfied in the premises;

            IT IS, THEREFORE, ORDERED that the defendant, __________________, be, and he is hereby, ordered extradited to the aforesaid County and State, and is remanded to the authorities thereof.

            IT IS FURTHER ORDERED that the officer in custody is directed to deliver the defendant to the duly accredited agent of the demanding state, together with a copy of this order and the waiver of extradition. The authorities of the demanding State take custody of the defendant not later than __________________.
